For the singluar, you'd certainly
write
duchess' hat
, but I think you'd say 'duchesses' hat. Just as you'd write
Jesus' hat
to be correct, but you'd probably say 'Jesuses' hat because it's easier to say and everyone would know what you meant.

So if you were talking about a minor feather in the hat belonging to the Duchess' Uncle James, you'd write "The duchess' Uncle James's' hat's feather was scarcely worth talking about" ?

Yes, I never intended that second ' to appear. The situation was made more complex because the Duchess's family always referred to Uncle James (who, actually was only visiting) as Unc'e Jam's (pronounced Unkie Jams)... so really should I have written:
"The duchess' Unc'e Jam's's (o'er from watching the football at St James' Park) hat's feather was scarcely worth talking about" ?
